The forms below can be printed and completed by parents. Some of these forms need to be taken to physicians for completion or filled out for the Health Aide to fax to your child's doctor. Some forms require a physician and parent signature. For any questions, please contact the Health Room at your child's school.
Please complete this form and turn it back into the office if you would like your child to receive basic health items from the Health Room if needed. (Vaseline, lotion, etc...)
